Our primary health care teams also understand the issues faced by people living in small rural communities, as well as urban settings. In fact, they have often faced them with you. You can also be confident that you are joining a patient-centred health provider that receives the best possible support to run their practice. Enrolment Process

How to enrol with a Waitaha Primary Health General Practice

We want to ensure that you have the ability to enjoy your best life with good health and wellbeing. One of the most important and positive steps you can take is enrolling with one of our General Practices.

Once enrolled, you will pay less than if you were making a casual visit. Click here to find out more about General Practice fees. If your child is under the age of 14 and enrolled with one of our General Practices, their visit will be free.

Enrolling when you and your whānau are healthy is the best time to take this step. If you, or someone in your family is sick, you can still contact one of our GPs to enquire about booking a casual visit.

Step 1: Take a moment to check you are eligible for publicly funded health care. Click here.

Step 2: If you’re eligible, download, print and fill in the form below. This will only take a few minutes.

  • The form asks you to fill in the name of your preferred General Practice. Click here to choose from our list of practices. Sometimes, a practice may not be taking any more patients. Before you enrol online, phone your preferred practice and check if they have the capacity to enrol you.
  • If you have been enrolled at another GP, we will ask for your permission to transfer your medical records from their practice to ours.

You will be asked to provide personal details such as your birth date and contact details. These details and your privacy are protected by the Privacy Act 1993.

Step 3: Please print off the enrolment form at the below link and take it in person to your chosen practice with your birth certificate or passport.
